In this paper, we provide a unique technique for detecting and classifying WBCs,RBCs, and platelets inside blood smear pictures using ResNet (Residual Neural Network), a deep learning architecture. Because of its capacity to efficiently train very deep neural networks while minimizing the vanishing gradient problem, the ResNet architecture has exhibited excellent performance in a variety of image recognition applications. Leveraging the power of ResNet, we developed a multi-class classification model capable of distinguishing between WBCs, RBCs, and platelets within microscopic images of blood smears. Our methodology involved preprocessing the blood smear images to enhance contrast and remove noise, followed by image segmentation to isolate individual blood cells and platelets. The segmented images were then used to train and fine-tune a ResNet model, utilizing a large annotated dataset of labeled blood cell images. The trained model exhibited remarkable accuracy in identifying and classifying different blood cell types, even in the presence of overlapping cells or artifacts. We extensively tested our suggested technique, on a range of blood smear images to evaluate its performance. The findings demonstrated that ResNet effectively identifies and categorizes WBCs, (RBCs) and platelets. When compared to methods our approach showcased superior accuracy, robustness and generalization capabilities. After training the model with the Resnet algorithm we got 92% of Accuracy. The research makes use of tailored retinal images together with the OPF (Optimum-Path Forest) and RBM (Restricted Boltzmann Machine) models to categorize images according to the presence or absence of DR. In this work, features were extracted from the retinal images using both the RBM and OPF models. In particular, after a thorough system training phase, RBM was able to extract between 500 and 1000 features from the images. The study included fifteen distinct trial series, each with thirty cycles of repetition. The research comprised 122 eyes, or 73 diabetic patients, with a gender distribution that was reasonably balanced and an average age of 59.7 years. Remarkably, the RBM-1000 model stood out as the top performer, with the highest overall accuracy of 89.47% in diagnosis. In terms of specificity, the RBM-1000 and OPF-1000 models surpassed the competition, correctly categorizing all images free of DR symptoms. These findings highlight the potential of machine learning, particularly the RBM model, for self-identifying illnesses. The potential of machine learning models—in particular, RBM and OPF—to automate the diagnosis of diabetic retinopathy is demonstrated by this work. The results show how well the RBM model diagnoses, how sensitive it is, and how well it can be applied for efficient DR screening and diagnosis. This information may be used to improve the effectiveness of systems that identify retinal illnesses. It represents a public health problem, with high prevalence in children, especially in tropical and subtropical areas. Factors such as poverty, lack of hygiene and limited access to drinking water favor its spread. In Latin America, the prevalence ranges between 20-80%, with an impact on child development. In Ecuador, it mainly affects children due to immunological immaturity and poor sanitary conditions. The aim is to know the prevalence in the Tachina parish to reinforce control strategies, promoting hygiene, sanitation and deworming campaigns to reduce infection and improve child health. Methods: The study has a cross-sectional, descriptive, and retrospective approach of anonymized data descriptively analyzed in SPSS 20.7. Results: 445 children from 0 to 10 years old were analyzed, with greater representation of the 5–10-year-old group (55%). Parasitosis affected 69% of cases, predominating in the 5-10 age group (67%). Polyparasitism was more frequent (59%) than monoparasitism (41%). Protozoa were the most common parasites, with Entamoeba coli standing out (30%). Among helminths, Ascaris lumbricoides was the most prevalent (59%). Conclusions: The results highlight the high parasitic load in children, with a predominance of polyparasitism, possibly due to inadequate hygiene habits or lack of access to drinking water. Therefore, it is recommended to reinforce education in personal hygiene and healthy habits in schools and communities. Older adults are at greater risk of self-medication because they tend to have multiple chronic conditions and the large number of medications they take can lead to drug-drug interactions. Objective: To describe the prevalence and characteristics of self-medication in adults aged 60 to 75 years in the 11th Municipality of the Autonomous City of Buenos Aires during the period between April and May 2024. Methods: Descriptive, cross-sectional and quantitative study. An ex profeso instrument consisting of 26 closed questions was used. Results: A total of 104 older adults aged between 60 and 75 years were interviewed. Most of them were female (60.58%) and had a chronic underlying disease (66.35%). The prevalence of self-medication was 74.04%. The ailments/pain that motivated self-medication were musculoskeletal pain (87.01%) and fever (63.64%), and consequently the most used drugs were analgesics (94.81%). When asked about the effects of the drugs, 98.70% reported improvements in their symptoms and generally no adverse reactions (80.52%). Conclusions: The high prevalence of self-medication among older adults is mainly due to difficulties in getting appointments for medical check-ups. Although most respondents reported improvements in their health and few adverse effects, these findings highlight the need to improve access to medical care and to promote responsible use of medicines. Although ENT infections are the main causes, odontogenic infections can also lead to ESD. Early diagnosis, intravenous antibiotic therapy and surgical intervention are essential to reduce mortality. Clinical case: A 32-year-old male patient presented with a month-long picture of swelling and pain on the left side of the face, recently aggravated by fever, headache, vomiting and generalised tonic-clonic convulsions. He had a history of alcoholism and recurrent dental infections. Imaging revealed a bilateral subdural empyema with left-sided predominance. Urgent antibiotic treatment was initiated, followed by bilateral craniotomy and drainage of purulent material. In addition, a brain abscess and a subgaleal haematoma were managed. Cultures identified Streptococcus viridans and coagulase-negative Staphylococcus, with good response to targeted therapy. The patient progressed favourably and was discharged in good condition. Conclusions: In regions like Bolivia, the prevalence of odontogenic infections due to cultural and economic factors increases the risk of severe complications such as SDE. A multidisciplinary approach, including early diagnosis, broad-spectrum antibiotics, and surgical intervention, is essential to improve outcomes and reduce mortality in these patients. Objective: to describe the different techniques and procedures associated with third molar surgery. Methods: a bibliographic review was carried out by searching for content on tooth retention and third molar surgery in SciELO, Redalyc, Scopus, PubMed, from articles published in international and open access journals. 40 studies were obtained, of which 32 were identified as appropriate for this research. Scientific articles, books and theses in English and Spanish, in full text, were included. Development: The eruption of third molars over the years has been one of the main causes of malocclusions, lesions, infections, cysts, tumors and pathological factors, which is why their excision is performed, and due to their proximity to the mandibular ramus and their proximity to the maxillary sinus. During the procedure, complications arise due to the need to perform an incision, osteotomy and odontosection, which are very traumatic, causing edema, bleeding, pain and trismus, in which it has been shown that the application of osteotomy techniques reduces the operating time and thus speeds up the avulsion of the third molars. Conclusions: The graduate profile of the Dentistry career establishes that general practice dentists must be prepared for the need for an extraction, and in many cases the surgical technique helps and facilitates, therefore, their training is important when performing an extraction. In addition to conventional treatment, there are different adjuvant therapies, including phytotherapy with Punica granatum. This plant has various properties, including its antioxidant, anti-inflammatory and antimicrobial power. Objective: To determine the effectiveness of using P. granatum as an adjuvant treatment for periodontal disease. Methods: A bibliographic review of scientific articles conducted in the last 10 years was conducted. Development: The use of P. granatum, or pomegranate, in the treatment of periodontal diseases has shown promising results. It has been shown that pomegranate mouthwash is effective in reducing the gingival index, bleeding on probing and plaque index compared to chlorhexidine. Its use significantly reduces bleeding and improves periodontal indices, suggesting an anti-inflammatory and antimicrobial effect. Furthermore, it was observed that the application of pomegranate extract gel also contributes to the reduction of gingival index and pocket depth in patients with gingivitis. Patients treated with pomegranate showed a remarkable improvement compared to those who did not receive this adjuvant. However, some participants reported adverse effects such as bitter taste and tooth staining. Overall, the beneficial properties of pomegranate are highlighted in the management of periodontal diseases. Conclusion: The use of P. granatum as an adjuvant for periodontal treatment demonstrates an anti-inflammatory and antimicrobial effect which can be used without producing adverse effects. Availability, attitudes and influencing factors Abstract: Introduction: Blood is a fluid of living cells, the quantity of which can be affected by various causes. Blood transfusions are essential in the treatment of multiple medical situations that cannot be treated with other procedures. They can occur as a result of voluntary, replacement and paid donations. This research was carried out with the aim of characterizing the blood donation process. Methods: a bibliography review was carried out, where 30 articles in Spanish and English were selected, published in the last five years on the subject, in databases such as: Scopus, Scielo, Pubmed, Springer. Results: blood donation is a social fact. The current shortage of blood donations is due to an inefficient blood supply system, in which replacement donation predominates, although voluntary donation is the most popular form of donation. This suggests a failure in primary health care and its promotional value, due to the lack of education and culture of donation in the world population. Donations are also affected by political, social and psychological aspects. Conclusions: the low number of donations worldwide determines the presence of the unavailability of blood for transfusion as one of the current health problems. Aristotle sees need as something inevitable, while Freud links it to the basic impulses of the id. Murray distinguishes between primary and secondary needs, which are essential for psychological well-being. Understanding these needs is crucial for human development. The aim of this study is to analyze the concept of health needs in nursing doctoral theses using Rodgers' evolutionary method. Methods: conceptual analysis based on Rodgers' Evolutionary Model. Data collection in the Portuguese Open Access Scientific Repository with descriptor "Health Needs" and subject "Nursing". Applied to doctoral theses with open access. The year of publication, country of origin, concept, attributes, antecedents, consequents, substitute terms and related concepts were evaluated. Results: Seven theses were selected. They address the concept of Health Needs presented in two pillars: 1) health strategies in a relationship of reciprocity between the health professional and the approximation of the care expected with that provided; 2) the dimensions of health gains (other concepts have emerged such as practice directed towards equity and family support). Conclusions: Health needs is a dynamic concept that is an integral part of the process of theoretical development in nursing and a central focus in determining care for the person being cared for or their family. In Latin America, Primary Health Care (PHC) policies have been promoted since 1986, despite challenges related to equity and sustainability. In Ecuador, following the COVID-19 pandemic, the Ten-Year Health Plan 2022-2031 was implemented as a public policy to establish objectives, strategies, and goals for improving population health. Aligned with this initiative, PUCESE has developed programs to promote health through its community outreach efforts. Method: This study employed a cross-sectional, descriptive, and retrospective analysis of data recorded in Excel, which included the total number of interventions and anonymized information about the project beneficiaries. Results: A total of 2,591 residents from northern communities in the province of Esmeraldas benefited from a single medical care intervention. This intervention identified key health conditions and facilitated the development of individualized and collective promotion and prevention measures tailored to the population’s needs. Conclusions: Community outreach initiatives strengthen public health policies and foster strategic partnerships to ensure access to basic services, reducing morbidity and mortality. Human-centered PHC enhances prevention and improves quality of life through integrated care networks. Moreover, promoting healthy habits and addressing social determinants of health are vital to combating poverty, inequality, and malnutrition, thereby contributing to the development of just and sustainable societies. It is the responsibility of rehabilitation nurses to develop strategies that respond to the specific needs of this ageing population, promoting their autonomy, independence and well-being in the community. Stroke has been identified as a public health problem and is one of the cerebrovascular pathologies that cause permanent disability. Afaf Meleis' theory of transitions offers a conceptual framework that allows a deeper understanding of the process of change and adaptation of people to the transition they experience, contributing to personalized care. It helps specialist nurses to understand and intervene in the transition processes experienced by people who have suffered a stroke. This article analyzes the application of this theory to the practice of rehabilitation nursing, highlighting the challenges and essential interventions for a successful transition from a medium-stay and rehabilitation unit to home. Depressive manifestations may arise as a result of the emotional impact of a chronic and potentially disabling condition, as well as from neurobiological changes associated with the disease or immunosuppressive treatment. Method: In this article, we report a case of a 51-year-old man diagnosed with MG and concomitant depressive symptoms. Treatment was initiated with sertraline 150 mg/day and trazodone 50 mg/day. Results: The choice of psychotropics should be made with caution, opting for drugs with a lower likelihood of affecting neuromuscular function. SSRIs are the most effective drugs for treating depression in patients with MG, and it is advisable to avoid drugs with anticholinergic effects, such as tricyclics and haloperidol. Conclusion: Caution is important when prescribing psychotropic medications to patients with MG due to the risk of affecting neuromuscular transmission. Case report Abstract: Introduction: immune thrombocytopenia (ITP) is an autoimmune disorder characterized by autoantibody-mediated destruction of platelets, resulting in low platelet count and increased risk of bleeding, and may be an initial manifestation of systemic lupus erythematosus (SLE), especially in patients with findings suggestive of autoimmunity. Clinical Case: a 52-year-old male presented with fever, hematuria, generalized petechiae (including mucous membranes), myalgia, mucosal bleeding, oliguria progressive to anuria, and severe arterial hypertension. Initial studies revealed severe anemia, severe thrombocytopenia, and acute kidney injury, which led to his admission to the Intensive Care Unit (ICU). Complementary studies showed positive ANA, low complement (C3, C4), and positive anti-Ro, while anti-dsDNA antibodies, ANCA, and markers of antiphospholipid syndrome were negative. Imaging revealed pulmonary edema with mild alveolar hemorrhage and renal ultrasound findings consistent with active lupus nephritis. These findings raised the possibility that severe thrombocytopenia was a prelude to SLE. After initial treatment refractory, the patient was escalated to rituximab, with a favorable response without complications, which allowed discharge from the ICU. Conclusions: this case highlights the importance of considering SLE in patients with refractory thrombocytopenia without apparent cause, even in men, and underlines the need for a multidisciplinary approach for timely diagnosis and treatment, thus preventing serious complications. Septic arthritis in the pediatric age group is a medical emergency with serious consequences if not treated early. Early diagnosis is essential to initiate appropriate treatment and prevent permanent joint damage. Ultrasound has emerged as an essential tool for the early and accurate detection of septic arthritis, allowing for a rapid and detailed evaluation of the affected joint, especially in cases where clinical findings are subtle. Clinical Case. A 1-month-old male infant was brought by his mother to the emergency service of our hospital due to irritability and crying when changing his diaper, significant increase in the volume of the right thigh, and pain during movement. An initial ultrasound was performed, which reported signs of superficial cellulitis, and a second ultrasound was performed, which revealed thickening of the synovium, a fluid collection in the joint capsule with echoes inside it, which concluded with the diagnosis of septic arthritis of the right hip. Discussion. Ultrasound not only allowed the identification of joint effusion and synovial thickening, but was also crucial in differentiating between superficial infection (cellulitis) and deeper septic arthritis. Conclusions. The importance of our clinical case lies in the use of ultrasound as a diagnostic method to detect joint involvement early. In combination with the clinical method and microbiological studies, it becomes an important component of the diagnostic approach and timely management of septic arthritis, thus reducing associated complications. The Rehabilitation Nurse Specialist plays an essential role in mitigating these effects through interventions aimed at the autonomy and independence of the person with Parkinson's Disease. Objective: Describe the benefits of implementing a rehabilitation nursing program for people with Parkinson's disease immobility syndrome in a community setting. Case Report: This is a descriptive case report of a person with immobility syndrome due to Parkinson's disease. After identifying the nursing diagnoses, a care plan was drawn up centered on functional rehabilitation, focusing on body balance, muscle movement and intolerance to activity, in a home context. The therapeutic plan included home visits three times a week, including therapeutic exercises, mobilizations, balance training and muscle strengthening, as well as training for the family/caregiver. Conclusion: After two and a half months of intervention, the rehabilitation nursing plan implemented for the person with advanced Parkinson's disease showed a positive evolution in mobility, muscle strength, body balance, increased functionality and, successively, independence for activities of daily living. The caregiver's burden was reduced, which was reflected in an overall improvement in the family's quality of life. In view of the motor and functional changes triggered by Parkinson's Disease, it is essential to implement a Functional Motor and Respiratory Re-education plan, associated with training strategies, which will maximize independence and improve the quality of life of the person and the caregiver. In addition, in recent decades the number of patients diagnosed with sepsis is growing every year. Objectives: to determine whether the implementation of corticosteroids in the basic treatment of patients who are in sepsis or suffering from septic shock brings benefits or harms. Method: this systematic review is based on a thorough search in the Medline (Pubmed) and Google Scholar databases, the analysis of the publications was chosen between the years 2010 to 2024. Result: according to the evidence found in the 20 articles reviewed, the use of corticosteroid therapy reduces the time in some patients who are in shock, mainly those who do not respond adequately to conventional vasopressin fluids and drugs. However, it is still under discussion whether these data can be interpreted as beneficial for patients in terms of mortality. And whether the benefits are the same for septic patients given the potential risks that corticosteroids cause to the immune system and the hyperglycemia evidenced in some studies. Conclusion: The results of the systematic review and the cohort studies analyzed suggest that there are still many disagreements on the matter, but most recommend the use of corticosteroid therapy as part of the treatment of patients with septic shock and not in sepsis. More than 50 % of those affected are pediatric, which makes recovery difficult because their tissues are in full growth. Objective: to determine the best treatment to recover the functionality of the burned hand, and minimize the consequences of contractures. Method: an exhaustive search was carried out where relevant works were selected for this study, for their treatment and reconstruction techniques, and the age of their patients. The variable of greatest consideration is the functionality of the hand. Results: in the case of pediatric hands, which have a greater propensity for poor healing, it is essential to evaluate the procedures and their reconstructive results in the acute phase to reduce surgeries due to complications. Patients are more concerned about their aesthetic appearance, especially children, and about their functionality, despite the pain they may feel. Conclusions: surgical treatment, performed by a team of specialists, is the best option to prevent and resolve scars and contractures that hinder hand functionality, as well as aesthetics and growth sequelae in pediatric patients.
