Its incidence has increased since the second half of the 20th century. The clinical-epidemiologic characteristics of these patients in Pinar del Río are unknown. Objective: to clinically and epidemiologically characterize chronic lymphocytic leukemia patients treated at the General Teaching Hospital “Abel Santamaría Cuadrado”, 1987-2020. Materials and Methods: a descriptive, longitudinal and retrospective study was performed. The clinical histories of 120 patients diagnosed with this disease were reviewed and the variables age, sex, clinical manifestations and clinical stage at diagnosis were used. Results: male patients older than 61 years predominated, general symptoms and stage IV were more frequent at presentation. Conclusions: the results do not differ from those reported by national reference centers and other international centers dedicated to the care of hematologic malignancies. It is likely that the advanced clinical stage at debut, found in most patients, is related to the delay in diagnosis. Objective: to assess the effectiveness of chromic catgut implantation at selected acupuncture points according to traditional Chinese syndromic diagnosis in men with idiopathic infertility. Methods: quasi-experimental study with pre-test-post-test and control group, in 135 patients with idiopathic infertility divided into: study group (67 treated with catgut implantation) and control group (68 treated with medication). A traditional medical history was taken and descriptive and inferential statistics were used. Results: mean age was similar in both groups (33,90 ± 6,28 years study group and 33,68 ± 5,6 years control group). Patients with Kidney yang vacuum predominated (55,2 % study group and 52,9 % control group). Both groups showed highly significant differences in sperm concentration, motility and morphology before and after treatments and between groups. Clinical improvement (64,2 % vs. 8,8 %), spermogram normalisation (53,7 % vs. 16,2 %) and pregnancy achievement (17,9 % vs. 4,4 %) were higher in patients treated with chromic catgut implantation compared to those using Clomiphene. Conclusions: chromic catgut implantation in selected acupuncture points according to traditional Chinese syndromic diagnosis in men with idiopathic infertility is effective because it improves sperm concentration, motility and morphology, normalises the spermogram and contributes to achieving pregnancies with increased fertility, to a greater extent than treatment with Clomiphene; together with the clinical improvement of the patients. There is a paucity of data on the knowledge and perception of healthcare workers regarding AI technology. This study aims to assess the knowledge and perception of healthcare workers towards the application of AI in healthcare services in Nigeria. Materials and methods: cross-sectional questionnaire-based survey design was used to achieve the aim of this study. Both electronic (Google form) and hardcopy version of the questionnaire were distributed to healthcare workers in Nigeria and their responses were retrieved and statistically analyzed. Results: out of 263 respondents, most 51,3 % (n=135) were females. Greater percentage 25,5 % (n=67) of the respondents were radiographers, followed by medical consultants 14,8 % (n=39) and the least 1,5 %(n=4) were pharmacists. Greater proportion 61 %(n=160) of the respondents have the opinion that AI can be incorporated into all medical specialties. Out of 263 respondents, 51,7 % (n=136) had good knowledge of AI and the least 6,4 % (n=16) had very poor knowledge of AI. Greater proportion 78,7 % (n=207) of the respondents, agreed that AI can help to reduce the number of medical errors. Majority 29,3 % (n=77) of the respondents agreed that human specialists will be replaced by AI in the near future. A large proportion 40,3 % (n=106) of the respondents agreed that some employers may prefer AI to human specialists because AI has no emotional exhaustion or physical limitation. Conclusion: the respondents in this study showed good knowledge of both the medical areas of applications of AI as well as the benefits of AI application in healthcare services. However, most of the respondents were afraid that their jobs would be taken over by AI in the near future. The aim of this study was to relate the variables Vigorexia and steroid use, with the practice of bodybuilding in a group of people attending training centers for recreational or aesthetic purposes in Argentina. In the present study we will work with an observational, descriptive, and mixed non-experimental research approach; through the application of a validated questionnaire (Adonis Questionnaire), with the purpose of gathering information on the perception of concern about physical exercise and body image. The units of analysis corresponded to the 20 participants residing in the Autonomous City of Buenos Aires, Argentina in the period from June to December 2020 who practiced physical exercise in sports facilities. Fifty percent of the respondents belonged to the female sex, and 40 % were aged between 25 and 31 years. The mean score of the Adonis Questionnaire was 7,45 points. The results of this study do not show any general impairment in relation to the overall score of the Adonis Questionnaire, where 50 % of the sample studied had scores corresponding to body image concerns that did not affect daily life. A detailed analysis of the individual results of the study shows that the subjects show good academic-work perception. One of the principal causes is Atrial fibrillation (AF) being the most frequent sustained arrhythmia represented by 160 000 patients who develop it annually in the United States. AF generates a turbulent flow within the left atrial appendage, which causes the formation of intracavitary thrombus with the risk that they are expelled into the systemic circulation, possibly causing a stroke as a complication. The purpose of this research study is to establish the prevalence of AF in patients with ischemic stroke older than 55 years old in the clinic service of the Hospital Alberto Antranik Eurknekian from January through September 2022. Material and methods: This research study was conducted following a non- experimental, descriptive, quantitative, transversal and retrospective approach. Data were gathered upon the clinical records of patients older than 55 years old, who had an admission electrocardiogram (ECG) and had ischemic stroke evidenced in topographies of the brain in Hospital de Ezeiza. The following variables were analysed: age and sex, arterial hypertension, diabetes, obesity, dyslipidaemia, smoking, history of previous stroke or transient ischemic attack, as well as AF and AF under treatment. Results: The final sample of this study consisted of 49 patients, of whom 55, 1 % were male (N=27) and 44, 9 % female (N=22). 8, 2 % (N=4) of them had associated atrial fibrillation, among them, 75 % (N=3) were anticoagulated. 77,6 % (N=38) had hypertension, 40, 8 % (N=20) diabetes, 6, 1 % (N=3) used tobacco and 4,1 % (N=2) had obesity and dyslipidaemia. 16,3 % (N=8) suffered an ischemic stroke previously and 2 % (N=1) had a transient ischemic attack. Conclusion: It was found a high prevalence of AF in patients diagnosed with a stroke. Of these, 75 % were anticoagulated, but still developed the ischemic event. Regarding cardiovascular risk factors, the entire sample had a high prevalence of comorbidities, the most common being hypertension and diabetes. It is important to predict its occurrence to guide postoperative management. There are few studies on the correlation between amylase levels in the drainage fluid and the risk of pancreatic fistula. The aim of the study was to determine whether the amylase level on postoperative day 3 can predict the severity of pancreatic fistula. Method: a retrospective study was performed in patients who underwent duodenopancreatectomy to evaluate the relationship between the amylase value in intra-abdominal drainage on postoperative day 3 and the severity of pancreatic fistula. The relationship between surgical and conventional biochemical results was analyzed. Student’s t-test was used for statistical analysis with a value of p<0.05 considered significant. Results: No significant differences or statistically significant correlations were found between the groups evaluated. Conclusion: Isolated measurement of amylase in draining fluid in the early postoperative period is not sufficient but is very useful in predicting the occurrence of FP associated with complications and prolonged hospital stay. Further research is required to reduce its incidence. Clarifying the enigma Abstract: Introduction: vampirism is the behavior of a person who acts like a vampire. Vampires are not real beings, but for two centuries there have been countless medical studies carried out to provide a causal and pathogenic response to the phenomenon. The exact disease that causes most of these vampiric alterations is porphyria. Objective: to describe the relationship between the classic signs and symptoms of the porphyric patient and the appearance and lifestyle of the mythical vampire. Method: a literature review was carried out during the period from August 1 to 20, 2023. Articles published in the last 5 years in Spanish and English were included and repeated or incomplete articles were excluded. The ScieELO, Medigraphic, and PubMed databases were reviewed. The search terms included Vampirism, Vampirism in Medicine, Porphyria, as well as their translation into English. Development: the association between vampirism and porphyria is probably the most famous. Erythropoietic congenital porphyria is the one associated with vampirism. It causes hypersensitivity to light and chronic hemolytic anemia. The similarities between the impressive description of the vampire's anatomy and the physical appearance of porphyria sufferers are more than evident. Conclusions: many diseases have been proposed as an explanation for the appearance and behavior of this enigmatic character, with porphyria being the most accurate. Photosensitivity, pale skin, erythroderma and dark clothing are distinctive features of the “bloodsucking monsters” and are consistent with the clinical expression of the aforementioned disease. The immune response is the mechanism with which the body reacts against foreign substances. Stress is the body's way of responding to any type of demand. One of the structures affected by stress is the thymus gland. Objective: to describe the influence of stress on the thymus and the immune response from a psychoneuroimmunoendocrinological perspective. Method: a search was carried out in the databases Scopus, PubMed, Medgrphic, Science Direct, Schoolar Google, Scielo, and in updated scientific books and journals. 26 bibliographies were selected, mainly from the last 5 years. Development: the sympathetic nervous system, the hypothalamic-pituitary-adrenal axis and the immune system with the thymus as one of its central organs, in integration with its components, are the material basis of the immune response. Psychological stress can alter the immune response through direct innervation of lymphoid organs. These effects have disastrous consequences for homeostasis, resulting in the appearance of diseases and the weakening of the immune response in periods of human ontogeny. Conclusions: stress causes lymphocytopenia, a decrease in monocytes, macrophages, mast cells, T cells, NK cells and involution of the thymus, which has a negative impact on thymopoiesis, causing immunosenescence and favoring the development of various pathologies. Psychoneuroendocrinoimmunological procedures constitute a range of therapeutic-diagnostic options for multiple diseases, from Medicina Alter. Genetic, immunological, environmental, psychological and endocrine factors are believed to contribute to its development. The diagnosis of RA is based on the confrontation of clinical manifestations and laboratory tests, with classification criteria established by the American College of Rheumatology (ACR) and the European League Against Rheumatism (EULAR). Treatment has evolved significantly in recent years, focusing on obtaining and maintaining clinical remission to prevent structural and systemic complications. Rehabilitation plays a crucial role in the management of RA, relieving pain, preventing deformities and improving quality of life. Physical therapy programs include joint mobility exercises, resistance exercises, therapeutic massage, aerobic exercises and stretching, among others. RA can have different patterns of evolution, such as acute, cyclic or progressive. Physiotherapy helps to improve the physical and psychological function of patients, adapting to the stages of the disease. In addition, a multidisciplinary approach including pharmacological treatment, orthopedic surgery when necessary and personal care is recommended. Rehabilitation is essential to maintain the functional independence of RA patients and improve their quality of life. Physical therapy programs should be tailored to the individual needs of each patient and consider the stage of the disease. Physiotherapy has been shown to reduce pain, improve muscle function and strength, and contribute to the overall well-being of RA patients. The need for an intersectoral perspective in health management is highlighted, recognizing that health problems go beyond the mere treatment of diseases. It discusses how primary health care (PHC) acts as the first point of contact with the health system, emphasizing the importance of comprehensive and person-centered care. In addition, the challenges and advances in the implementation of public health indicators in the context of SUS are examined, highlighting the need for effective management and continuous evaluation of system performance. Obstacles related to decentralization and regionalization of health services are discussed, as well as the importance of community participation and empowerment in health promotion. In this regard, the relevance of close collaboration between academia and administration to develop effective strategies to improve the quality of health services is emphasized. This analysis underlines the importance of health indicators as fundamental tools to guide public policies, evaluate system performance, and guarantee equitable and universal access to quality health services. Normally the events that set it in motion are those that are linked to variations that demand too much care from the person and therefore put their individual well-being at risk. Selye was the one who initially originated the somatic stress paradigm. He identified three stages or phases in the organic-vital response to stress, being factors of the 'General Adaptation Syndrome' The three stages are: alarm reaction, resistance and exhaustion. Regarding work stress, it is a pattern of reactions that originates when workers face work demands that are not equivalent to their expectations; Stressors can be present in any work sector, in any position or level and in any circumstance in which the person or load to which they cannot adapt quickly may be subjected. Memory is defined as the ability to retain, consolidate, and retrieve data. There are different areas of the brain involved in this function and that events such as stress can change their functionality. It was first named by psychiatrist Harrison G. Pope in the 1990s when studying weightlifters in a Boston gym. Vigorexia leads individuals to perceive themselves as small and weak despite having a muscular appearance. Some experts view Vigorexia as a behavioral addiction, marked by an obsession with exercise and the consumption of substances like anabolic steroids to increase muscle mass. It is often associated with distorted body image and dissatisfaction with one's appearance. The article highlights that Vigorexia has not always been recognized as a distinct disorder and was not initially included in diagnostic manuals. Anabolic steroids, testosterone, and growth hormones are commonly abused substances among individuals with Vigorexia, contributing to physical and mental health problems. Long-term use of these substances can lead to severe health issues, including psychosis. The article also touches on the impact of the COVID-19 pandemic on recreational bodybuilding practitioners, as lockdowns and social distancing measures disrupted their routines, potentially exacerbating anxiety and sadness. Additionally, it distinguishes between general physical activity and bodybuilding, with the latter emphasizing muscle development, strength, and aesthetics. The practice of bodybuilding often involves weightlifting and specialized machinery in gyms. In summary, Vigorexia is a mental disorder characterized by an obsession with muscularity, distorted body image, and the abuse of substances like anabolic steroids. It has physical and mental health consequences and has become more prevalent in recent years, affecting both athletes and non-athletes. It was first described by Virginia K. Frantz in 1959, but it was included in the World Health Organization (WHO) classification of pancreatic tumors in 1996 as a “pseudopapillary tumor” of the pancreas. Of unknown etiology, its incidence is observed in young women; Clinically it manifests with slow growth of a solid abdominal mass accompanied or not by abdominal pain. There has been a gradual increase in the incidence of this tumor in the last two decades, probably due to the greater frequency of the use of cross-sectional images in the evaluation of abdominal symptoms. Its degree of malignancy is low and complete resection allows the patient a favorable evolution and prognosis. Materials and Methods: a retrospective observational bibliography review of patients with PNS operated on from October 2016 to December 2022 was carried out. Results: the patients operated on for Frantz tumor analyzed in the study did not have recurrences and/or metastases. Conclusion: pseudopapillary neoplasia of the pancreas (Frantz's tumor) should be considered in the differential diagnosis of pancreatic tumors, especially in young women, since surgical treatment has a good prognosis even if the tumors are large. It is clinically characterized by the presence of predominantly nocturnal pruritus that is palmoplantar at first and then generalized. Alterations in liver function with or without jaundice and elevated serum bile acid levels, biochemical cholestasis is classified as mild to moderate in severity. This pathology does not have significant consequences for the mother, but it is associated with a high risk of affecting the newborn, such as premature delivery, fetal distress, fetal arrhythmias and intrauterine death. Objective: diagnostic methods and behaviors in their pharmacological treatments that provide the best results for those patients. Methods: systematic review with qualitative database from the PUBMED platform. The articles were downloaded from the PUBMED platform and Zotero was used to make the proper citations and references. In the investigation, the population will be composed of adult female patients who were admitted to any health care unit during the second and third trimesters. Results: the PUBMED database was used with the following MESH terms (cholestasis of pregnancy) that generated 2 597 results. 28 articles were analyzed in more depth, of which 12 articles were finally selected for the final analysis. Ursodeoxycholic acid (UDCA), a beta epimer of chenodeoxycholic acid, is a more polar bile acid than primary bile acids, with substantial choleretic and multiple other chemoprotective effects. The mechanism of action is not fully understood, but studies have shown that after treatment there is a reduction in total serum bile acids in both maternal and umbilical cord serum and a qualitative change in bile acid pool. serum. When compared to all controls, UDCA was also found to be associated with a decreased incidence of preterm birth, neonatal respiratory distress, and the number of neonatal intensive care unit admissions. Conclusion: the treatment of cholestasis gravidarum is the oral administration of ursodeoxycholic acid, it is effective and safe in the treatment of patients with cholestasis gravidarum. The most widely accepted hypothesis today is that excess catecholamines cause calcium overload in cardiac myocytes, which leads to interruption of contraction and ventricular function. Material and methods: a synthesis of the available evidence about takotsubo syndrome was carried out through a systematic review type study and the maximum number of articles that reported cases of treat-ment of this entity was included. For this, specific searches were launched with MESH type keywords. Results: 12 articles were selected. This included a total of 351,124 patients with Takotsubo syndrome, with a mean of 70,224 per study, of whom 5,82 % (20,418 of 351,124) were complicated by cardiogenic shock. Among the risk factors associated with cardiogenic shock were found: Use of drugs such as co-caine, peaks of emotional stress, physical stress from surgeries, menopause, hypoglycemia, massive blood loss, hypotension and use of drugs such as atropine, adrenaline, dobutamine, ergonovine, nortripty-line. Conclusion: from the analysis of the results obtained, physical and mental stress are risk factors associ-ated with cardiogenic shock in patients with Takotsubo syndrome, this corresponds to the most accepted hypothesis today that justifies this syndrome with the excess in the production of catecholamines, hor-mones secreted under stress. The objective of the present study is to systematically review and analyze the most current scientific evidence regarding the association between obesity and the development of breast cancer in premenopausal and postmenopausal women. Material and methods: a systematic review of the literature was carried out using a highly sensitive search in the main databases of Pubmed, Embase, Scholar google and Cochrane using observational studies and clinical trials that have been studied as inclusion criteria Obesity as a risk factor for breast cancer. Results: obesity and overweight are associated with a greater risk of breast cancer development, specifically ER+, a worse prognosis, and a greater mortality rate. The association between body fat, early age and breast cancer is contradictory, but can play a protective role in both pre and postmenopausal women. Conclusion: it is possible to infer that with regard to body weight, the best way to prevent breast cancer is to maintain an adequate weight, mainly through a healthy lifestyle. However, it is necessary to define, through studies, the amount of body weight gained and the period of life in which the risk of breast cancer increases. Method: a systematic review of the literature available in different databases was carried out, focusing on PUBMED. Selecting those articles that were best related to the proposed topic. Results: non-specific symptoms such as fever, cough, diarrhea, sore throat are the most frequent and that when the patient presents atypical symptoms such as anosmia or ageusia, which are more frequent in adults, the probability of a positive diagnosis is greater. Even if the pathology is more benign, there is also the probability that these same patients present some complication, such as pediatric Multisystem Inflammatory Syndrome (SIM-p), which has a wide spectrum of manifestations and which, if not diagnosed and treated, leads the patient to death. Conclusion: according to published studies, the symptoms presented are very non-specific and not very serious, so it is not a pathology that frequently carries a risk to health, even though some cases have an evolution that can develop complications and very rarely to death. The main mechanism is the fall on the outstretched hand, followed by sports injuries and road accidents. The complications associated with the fracture can cause joint limitation or chronic pain that can limit the patient's life, which represents a great challenge for its management. Material and method: this systematic review aimed to identify which approach has the best results in patients with distal radius fractures. Medline and Google Scholar were used as search bases, selecting potential works and important information. Of the 795 papers identified, 57 potential articles were selected. After applying the inclusion and exclusion criteria, 15 works were selected, dividing the patients into 2 groups: those treated surgically and those treated conservatively. Results: the results showed that surgical treatment presents a faster rate of consolidation and a potential prevention of nonunion compared to conservative treatment. However, surgical treatment also presented more complications. On the other hand, conservative treatment appears to be safer and is associated with a lower rate of complications. Conclusion: surgical treatment accelerates the return to work or previous activities earlier and to a better functional status, less pain and greater patient satisfaction compared to conservative treatment. However, conservative treatment appears to be safer and is associated with a lower complication rate. Therefore, the choice of approach to follow must be individualized for each patient, taking into account factors such as age, the severity of the fracture, and comorbidities. It is important to raise awareness about early treatment to prevent the high rate of morbidity and mortality. Methods: this is a systematic review that includes concepts and definitions of Fournier's gangrene, studies related to diagnosis, and emphasizes the importance of early treatment. Thorough searches were conducted in major databases such as PubMed, Ovid MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials. Specific search terms were used, such as "Fournier's gangrene," "debridement," "necrotizing fasciitis," "necrotizing soft tissue infections," and "necrotizing," employing the controlled language MESH in PubMed to ensure precise selection of relevant studies. Results: initially, 176 relevant articles published between 2012 and 2022 were found. After applying inclusion and exclusion criteria, 16 articles were selected, of which 13 were considered relevant for this study. The range of participants in the studies varied from 1 to a maximum of 3224 patients, with a total of 5860 patients included in the analysis. It is important to note that in all the studied articles, a decrease in the morbimortality rate was observed compared to the current mortality rate, which ranges from 20 % to 40 %. Conclusion: to reduce the morbidity and mortality associated with Fournier's gangrene, early diagnosis and prompt surgical treatment, including aggressive surgical debridement, are essential. These infections represent an important cause of morbidity and mortality in hospitalized patients. The implementation of prevention packages, including measures such as hand hygiene and the application of sterile barriers, has been shown to reduce the incidence of these infections. Other risk factors include the length of the catheter, the patient’s comorbidities, and compliance with prevention measures. Method: the study design was that of a systematic review of the literature. Selected articles from the specific search that met the inclusion criteria were analyzed, except for articles that met some exclusion criteria. Results: average percentage of infection every 100 days = 0,64 %. Median percentage of infection every 100 days= 0,42 %. Conclusion: in conclusion, it is important to implement effective prevention measures in the insertion and management of central venous catheters. These measures include hand hygiene, proper preparation of the insertion site, education and training of medical personnel, and the use of protective barriers. By consistently applying these measures, the incidence of CVC-associated infections can be significantly reduced and patient safety and care improved.
